All forms need to be submitted to Great Books by May 4th.

Physical Form: This form is not complete without the signatures of the camper’s physician and parent/guardian. Be sure to allow adequate time to complete this step with your doctor.  Please keep a copy of the medical forms for yourself before you return them to camp.

Immunization Record: This form must be completed by your child’s physician. Please be certain that all immunizations are current. We will be inspected weekly by the State Health Department and students who are not current will need to get boosters from our on-call doctor at your expense or will be sent home from camp.

Medication Authorization: This gives our Health Supervisor permission to administer both prescription and non-prescription medications to your child (please note that, in accordance with California and  Massachusetts state law, no student will be allowed to self-administer medication, with the exception of inhalers, Epi-pens, and insulin injections).

Permission Form to Carry and Self-Administer Emergency Medication: This form must be completed for any camper who must carry medication throughout the day. This would include medications such as inhalers and epi pens.

Health Information Sheet: This form provides us any updated medical information about your child that is not covered on the medical form and is very important to have on file with our health provider on campus.

Housing Information: This form explains our lost key policy and contains an optional section for housing requests.

Permissions: Please be sure that you and your child sign the necessary sections of this form.

Transportation Form: This form needs to be completed for all campers regardless of the need for us to transport your child either to or from camp. It needs to be returned to our office no later than 30 days prior to arrival.

Behavior Expectation: This ensures that all participants understand the basic rules of behavior while attending the program. We ask that both students and parent sign the form as indicated.

Additional Materials: To complete your child’s camp records, please send a current photo of your child so that we have one on file for identification and  emergency purposes and also to help our staff in greeting you child on campus or at the airport at the start of camp.
